What is nexium?
Nexium mups contains the active ingredient esomeprazole, a proton pump inhibitor (PPI) of the newer generation. Esomeprazole is the S-enantiomer (mirror image molecule) of omeprazole and has a somewhat better bioavailability and more uniform action.
The MUPS technology (Multiple Unit Pellet System) consists of many small skin juice resistant balls in one tablet. These can be dissolved in water, which makes it easier to take with swallowing problems.
Nexium is produced by AstraZeneca and is approved for the treatment of acid-related diseases. Generika with esomeprazole are also available.
Active Ingredients & Mechanism of Action
Active ingredient: Esomeprazol
Esomeprazole inhibits the proton pump of the stomach cells:
Active mechanism:
- Identical with other PPIs: irreversible inhibition of H+/K+-ATPase
- Blocking of gastric acid production in the last step
Advantages of Esomeprazole:
- Higher bioavailability as omeprazole
- More uniform metabolism (less dependent on CYP2C19 genetics)
- A little stronger acid inhibition in the first days
Clinical relevance: The difference to other PPIs is small. In some studies, Esomeprazole shows a somewhat faster healing in severe reflux esophagitis.
Who is it suitable for?
Nexium is suitable for:
- Gastroesophageal reflux disease (GERD)
- Refluxesophagitis (healing and relapse prophylaxis)
- Gastrointestinal ulcers
- Helicobacter-pylori-Eradikation
- Gastric protection at NSAID intake
- Zollinger-Ellison Syndrome
Especially suitable for:
- Heavy reflux esophagitis (Los Angeles Grade C/D)
- Swallow weights (MUPS in water dissolving)
- Patients with variable CYP2C19 activity
Available Dosages
Available starches:
- 20 mg MUPS tablets
- 40 mg MUPS tablets
Other dosages:
- Sodburning: 20 mg 1x daily
- Refluxesophagitis: 40 mg 1x daily for 4-8 weeks
- Maintenance therapy: 20 mg 1x daily
- Gastric protection at NSAID: 20 mg 1x per day
- Helicobacter Eradication: 20 mg 2x daily (with antibiotics)
How to Take
Intake:
- Morning, at least 30 minutes before dinner
- Swallow tablet with water
- Do not chew or crush
In case of swallowing complaints:
- Dissolve tablet in half a glass of still water
- Stirring until the pellets dissolve (tablet does not decompose completely)
- Drink within 30 minutes
- rinse glass and drink residues
About gastric probe: Nexium mups can be administered in water dissolved via a stomach probe.
Contraindications
**Nexium must not be taken at:* *
- hypersensitivity to esomeprazole or other PPIs
- At the same time taking Nelfinavir or Atazanavir
Preview at:
- Heavy liver failure (max. 20 mg)
- Long-term application (risks like all PPIs)
- Suspicion of stomach cancer
Pregnant period: Limited data. Use only if clearly indicated. Omeprazole has more experience data in pregnancy.
Possible Side Effects
Acid (1-10%):
- Headaches
- stomach pain
- diarrhea
- Blowing
- nausea
Occasionally:
- Blocking
- Mouth dryness
- Squeeze
- Sleep disorders
- Increased liver enzymes
Long-term risks (class effect of all PPIs):
- Magnesium deficiency
- Vitamin B12 deficiency
- Increased fracture risk
- Possible risk of kidney disease
Interactions
Contracted:
- Atazanavir, Nelfinavir (HIV drugs)
Esomeprazole inhibits CYP2C19 (moderate):
- Clopidogrel: Possible weakening of action (pantoprazole if desired)
- Diazepam: Slow down
- Phenytoin: Increased Mirror
Reduced recording:
- Ketoconazole, Itraconazole
- Iron preparations
- Erlotinib
