What is Femoston?
Femoston is a combined hormone replacement medication that brings together estradiol (estrogen) and dydrogesterone (progestogen) in one tablet. It is designed for women with a uterus who need both hormones.
Femoston is available in different variants:
- Femoston 1/10 and 2/10 (sequential: first estrogen alone, then the combination)
- Femoston conti (continuous: the same dose throughout)
The sequential variant leads to withdrawal bleeding; the continuous variant ideally leads to no bleeding.
Active Ingredients & Mechanism of Action
Active ingredient: Estradiol, Dydrogesteron
Estradiol:
- Replaces the missing estrogen
- Relieves menopausal symptoms
Dydrogesterone:
- Protects the uterine lining from overstimulation
- Offsets the increased risk of overgrowth and cancer of the uterine lining caused by estrogen alone
- However, combined HRT with estrogen and progestogen increases the risk of breast cancer, depending on how long it is used

Contraindications
Breast cancer, estrogen-dependent tumors, thrombosis, unexplained bleeding, severe liver disease.
Possible Side Effects
Breast tenderness, headache, nausea, bleeding, weight changes.
Interactions
Medicines that can weaken the effect of Femoston:
- Certain epilepsy medicines (e.g. phenobarbital, phenytoin, carbamazepine), certain medicines against infections (e.g. rifampicin, rifabutin, nevirapine, efavirenz), ritonavir and nelfinavir, and St. John's wort preparations can speed up the breakdown of the hormones. This can weaken the effect and change the bleeding pattern.
Effect on other medicines:
- Femoston is expected to lower the level of the epilepsy medicine lamotrigine. This can worsen seizure control.
Hepatitis C medicines:
- Caution is required when used at the same time as ombitasvir/paritaprevir/ritonavir and dasabuvir (with or without ribavirin), glecaprevir/pibrentasvir or sofosbuvir/velpatasvir/voxilaprevir.
Frequently Asked Questions
Sequential: bleeding at the end of each cycle. Continuous: the goal is no bleeding, but spotting is possible at first. Conti is suitable for women who have been in menopause for quite some time.
