Duac Akne Gel

Duac Akne Gel is a prescription-only medication for the external treatment of acne vulgaris. It contains two active ingredients whose effects complement each other: the antibacterial agent benzoyl peroxide and the antibiotic clindamycin.

The gel is applied once daily to the affected areas of skin and fights both acne bacteria and clogged pores.

Duac is manufactured by GlaxoSmithKline and is approved for the treatment of mild to moderate acne vulgaris.

The two active ingredients in Duac work through different mechanisms:

Benzoyl peroxide (5%):

• Has an antibacterial effect by releasing oxygen
• Kills the acne bacterium Propionibacterium acnes
• Has a mild peeling (keratolytic) effect
• Helps open clogged pores
• No resistance develops

Clindamycin (1%):

• Antibiotic from the lincosamide group
• Inhibits bacterial protein synthesis
• Reduces the inflammatory response
• Also has an antibacterial effect against P. acnes

Synergistic effect: Benzoyl peroxide reduces the development of antibiotic resistance to clindamycin.

Duac Akne Gel is suitable for:

• Adolescents and adults with mild to moderate acne vulgaris
• Patients with inflammatory forms of acne (papules, pustules)
• As first- or second-line therapy for acne

Particularly suitable for:

• Inflammatory acne with red pimples
• Combination skin with clogged pores
• When other topical treatments do not work sufficiently

Less suitable for:

• Very dry or sensitive skin
• Severe cystic acne (systemic therapy is often needed here)

Duac must not be used in cases of:

• Known hypersensitivity (allergy) to clindamycin, lincomycin, benzoyl peroxide or any of the other ingredients of the gel

Active ingredient concentration:

• Benzoyl peroxide: 50 mg/g (5%)
• Clindamycin: 10 mg/g (1%)

Pack sizes:

• 15 g tube
• 30 g tube
• 60 g tube

Frequency of use: Once daily, preferably in the evening before going to bed.

Application: 1. Wash your face thoroughly with a mild cleanser 2. Let the skin dry (wait approx. 10-15 minutes) 3. Apply a thin layer of gel to the affected areas 4. Treat the entire affected area, not just individual pimples 5. Allow to absorb completely

Timing: Apply once daily in the evening. When applied in the morning, the gel combined with sunlight can irritate the skin more.

Duration of treatment: Use should be limited to a maximum of 12 weeks to avoid antibiotic resistance. If needed, treatment can then be continued with a non-antibiotic product.

Important notes:

• Avoid contact with the eyes, mouth, and mucous membranes
• Benzoyl peroxide can bleach textiles (towels, pillowcases, clothing)
• Do not use other irritating products at the same time
• Use sun protection during the day

Duac must not be used in cases of:

• Hypersensitivity to benzoyl peroxide, clindamycin, or lincomycin
• History of antibiotic-associated colitis (pseudomembranous colitis)
• Application to open wounds or damaged skin
• In the area of the eyes, mouth, and mucous membranes

Caution in cases of:

• Very sensitive or atopic skin
• Concurrent use of other topical acne medications
• Tendency toward eczema
• Pregnancy and breastfeeding (only after consultation)

Not in combination with:

• Erythromycin (topical or systemic) – may cancel out the effect
• Other topical retinoids without a doctor's instruction

Very common (>10%):

• Dry skin
• Skin peeling
• Erythema (skin redness)
• Burning or stinging during application

Common (1-10%):

• Itching
• Contact dermatitis
• Worsening of acne at the beginning (initial flare-up)
• Sunburn-like reactions

Uncommon:

• Allergic skin reactions
• Swelling

Rare:

• Pseudomembranous colitis (with prolonged use over large areas)

Managing side effects: If skin irritation is severe, reduce application to every other day or pause temporarily. A non-comedogenic moisturizer can help.

Do not combine with:

• Erythromycin (topical or systemic) – antagonistic effect
• Topical isotretinoin – increased skin irritation
• Other topical antibiotics without consultation

Caution when used at the same time as:

• Other topical acne medications (adapalene, tretinoin) – irritation may increase
• Alcohol-containing products – increased drying
• Peels or abrasive products

Recommendation: If retinoids are to be used at the same time, it is best to apply them at different times (e.g., retinoid in the morning, Duac in the evening) or as directed by a doctor.
