Contraindications
Tadalafil ratiopharm must not be taken in cases of:
- use of organic nitrates in any form
- concomitant use of guanylate cyclase stimulators such as riociguat
- heart disease for which sexual activity is not advised
- a heart attack within the previous 90 days
- unstable angina pectoris or angina pectoris that occurred during sexual activity
- heart failure of NYHA class II or higher within the last 6 months
- uncontrolled heart rhythm disorders, low blood pressure (below 90/50 mmHg) or uncontrolled high blood pressure
- a stroke within the previous 6 months
- loss of vision in one eye due to non-arteritic anterior ischaemic optic neuropathy (NAION)
- hypersensitivity to tadalafil or any of the other ingredients
Possible Side Effects
Headache and back pain, heartburn, flushing.
Interactions
Do not combine with:
- organic nitrates in any form: Tadalafil enhances their blood-pressure-lowering effect. If nitrate administration is medically necessary in a life-threatening situation, at least 48 hours should have passed since the last dose of tadalafil; it should then only be given under close medical supervision.
- Riociguat: The combination can lead to a symptomatic drop in blood pressure.
- other PDE-5 inhibitors or other treatments for erectile dysfunction.
Not recommended or only with caution:
- Doxazosin: The blood-pressure-lowering effect is considerably enhanced (for at least 12 hours), and fainting (syncope) is possible. The combination is not recommended.
- Other alpha blockers (e.g., alfuzosin, tamsulosin): particular caution, especially in older people.
- Blood-pressure-lowering medicines: Tadalafil can cause an additional, generally slight, drop in blood pressure.
- CYP3A4 inhibitors (e.g., ketoconazole, ritonavir, saquinavir, itraconazole, erythromycin, clarithromycin) and grapefruit juice: They can increase the level of tadalafil in the blood; side effects can then occur more often.
- CYP3A4 inducers (e.g., rifampicin, phenobarbital, phenytoin, carbamazepine): They can lower the level of tadalafil and weaken its effect.
