How to Take
30-60 minutes before sexual intercourse, not more than once a day. The recommended maximum dose is 100 mg.
Contraindications
Sildenafil HEXAL must not be taken in cases of:
- concomitant use of nitrates or nitric oxide donors (e.g. amyl nitrite)
- concomitant use of guanylate cyclase stimulators such as riociguat
- conditions for which sexual activity is inadvisable (e.g. severe cardiovascular diseases such as unstable angina pectoris or severe heart failure)
- a recent stroke or heart attack
- low blood pressure (below 90/50 mmHg)
- severe hepatic impairment
- loss of vision in one eye due to non-arteritic anterior ischaemic optic neuropathy (NAION)
- hereditary degenerative retinal diseases such as retinitis pigmentosa
- hypersensitivity to sildenafil or any of the other ingredients
Possible Side Effects
Very common (at least 10 in 100 people treated):
- Headache
Common (1 to fewer than 10 in 100 people treated):
- Facial redness (flushing), hot flashes
- Digestive complaints (dyspepsia), nausea
- Dizziness
- Stuffy nose
- Visual disturbances, changes in color vision, blurred vision
Rare (1 to fewer than 10 in 10,000 people treated), including:
- Heart attack, unstable angina pectoris, heart rhythm disorders, sudden cardiac death
- Stroke, seizures
- Damage to the optic nerve with loss of vision (NAION), blockage of a retinal blood vessel
- Deafness
- Prolonged erection or persistent erection (priapism)
- Severe skin reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis)
Other side effects can occur uncommonly or rarely.
If you experience a sudden visual disturbance of any kind, stop taking Sildenafil HEXAL and see a doctor immediately. If an erection lasts longer than 4 hours, seek medical help immediately. Priapism that is not treated immediately can damage the tissue in the penis and lead to permanent loss of potency.
Interactions
Do not combine with:
- Nitrates or nitric oxide donors (e.g. amyl nitrite): sildenafil enhances their blood-pressure-lowering effect.
- Riociguat: the combination can lead to a symptomatic drop in blood pressure.
Not recommended or only with caution:
- Ritonavir: concomitant use is not recommended; in any case, no more than 25 mg sildenafil within 48 hours.
- Other CYP3A4 inhibitors (e.g. ketoconazole, itraconazole, erythromycin): consider a starting dose of 25 mg.
- Alpha blockers (e.g. doxazosin): a drop in blood pressure when standing up is possible. Alpha-blocker therapy should be stable beforehand; consider a starting dose of 25 mg.
- Nicorandil: because of its nitrate component, a serious interaction is possible.
- Sacubitril/valsartan: a greater drop in blood pressure is possible.
- Other PDE-5 inhibitors, other medicines containing sildenafil (e.g. for pulmonary arterial hypertension) or other treatments for erectile dysfunction: the combination is not recommended.
- Grapefruit juice: may slightly increase sildenafil levels in the blood.
