What is eletriptan?
Eletriptan is a second-generation triptan, which is characterized by a high efficiency even in heavy migraine attacks. It has good CNS penetration and often shows a better effect in studies than Sumatriptan.
The drug is particularly interesting for patients where other triptans do not have sufficient effect. The compatibility is good, and the side effect rate is comparable to other triptans.
The well-known brand name is Relpax®.

Active ingredient: Eletriptan
Eletriptan acts as a highly selective serotonin agonist:
Active mechanism:
- High affinity for 5-HT1B/1D receptors
- constriction of extended cranial vessels
- Inhibition of CGRP release
- Blocking trigeminal pain signals
Special features:
- High selectivity for relevant receptors
- Good lipophilia – penetrating well into the CNS
- Consistent action in severe attacks
Initiation of action:
- 30 minutes to 1 hour
- duration of action: 24 hours
Who is it suitable for?
Eletriptan is suitable for:
- Adults with acute migraine attacks
- Patients in which other triptans do not work enough
- Heavy migraine attacks
Especially suitable for:
- Non-responders to other triptans
- Frequent return of headaches
- Demand for reliable effect
Not suitable for:
- cardiovascular diseases
- Heavy liver failure
- Simultaneously taking strong CYP3A4 inhibitors
Available Dosages
Tablets:
- 20 mg
- 40 mg (standard dose)
Recommended dosage:
- Standard dose: 40 mg
- At lower response: try 80 mg
- Repeat after 2 hours
- Maximum: 80 mg in 24 hours
How to Take
Intake:
- Take the start of headaches
- Suck with water
- Independent meals
- Not during the aura
Time:
- Early intake improves the effect
- Return: Repeat after 2 hours
- If first dose is ineffective: No second for the same attack
Contraindications
**Eletriptan must not be taken at:* *
- coronary heart disease, vasospasms
- Heart attack, stroke, TIA in history
- Peripheral arterial disease
- Heavy or uncontrolled hypertension
- Heavy liver failure
- Hemiplegic or Basilaris marshes
- Simultaneously taking strong CYP3A4 inhibitors (Ketoconazole, Itraconazole, Clarithromycin, Ritonavir)
- Ergotamine intake (24h distance)

Possible Side Effects
Acid (1-10%):
- Squeeze
- Bemonia
- Tiredness, asthenia
- Paresthesia
- Headaches
Triptane-specific sensations:
- Warmth
- Closeness in breast/neck
- Crawling, numbness
- Pressure feeling
Occasionally:
- Heart palp
- Hypertension
- Mouth dryness
- stomach pain
Selten:
- Coronary seeds
- Allergic reactions
Interactions
Contraint with:
- Strong CYP3A4 inhibitors (Ketoconazole, Itraconazole, Clarithromycin, Ritonavir, Nelfinavir)
- Ergotamine (24h distance)
- Other triptans on the same day
Preview at:
- Moderate CYP3A4 inhibitors (erythromycin, verapamil, fluconazole)
- SSRI/SNRI (Serotoninsyndrom)
- Propranolol (light increase in eletriptan level)
