What is ceftriaxone?
Ceftriaxone is a broad spectrum antibiotic from the group of cephalosporins (3rd generation). It is used to treat various bacterial infections, including gonorrhea (tripper).
In Gonorrhoe, Ceftriaxon is the means of choice, as many gonococci strains have become resistant to other antibiotics. The treatment is carried out as a single injection, often in combination with oral aziithromycin.
Ceftriaxon must be administered by medical professionals.
Active Ingredients & Mechanism of Action
Active ingredient: Ceftriaxon
Ceftriaxone acts bactericidal by inhibiting cell wall synthesis:
Active mechanism:
- Ties to penicillin-binding proteins (PBPs) of the bacteria
- inhibits cross-linking of the peptideoglykan
- The bacterial cell wall becomes unstable
- The bacteria burst and die
Advantages of Gonorrhoe:
- Very high effectiveness against gonococci
- One-off treatment sufficient
- High tissue concentrations
- Low resistance rates
Pharmacokinetics:
- Long half-life (approx. 8 hours)
- Good penetration into all tissues
- partial renal, partly biliary excretion
Who is it suitable for?
Ceftriaxone is suitable for:
- Uncomplicated Gonorrhoe (genital, pharyngeal, rectal)
- Gonorrhoea in pregnancy
- Disseminated gonococci infection
- Other serious bacterial infections
Standard treatment Gonorrhoe: Ceftriaxone 1 g i.m. (intramuscular) unique PLUS Azithromycin 2 g oral unique
Not suitable for:
- Heavy penicillin allergy (crease allergy possible)
- Neonatal with simultaneous calcium infusion
Available Dosages
Gonorrhoe – simple:
- 1 g intramuscular as a single dose
- Plus Azithromycin 2 g oral
Gonorrhoe with complications:
- 1-2 g intravenous every 24 hours
- For 7-14 days
Available starches:
- Powder for producing an injection solution: 250 mg, 500 mg, 1 g, 2 g
** The injection takes place in a large muscle group (usually buttocks). If >1 g is added, divide into two injection points.
How to Take
Depletion: Ceftriaxone is administered as an injection by medical professionals. It is not suitable for self-application.
For Gonorrhoe:
- A single injection into the butt muscle
- Together with oral aziithromycin (2 g single dose)
- Recheck after 2 weeks recommended
After treatment:
- Sexual abstinence for 7 days
- All sexual partners of the last 60 days inform and treat
- Test for more STI recommended (HIV, Syphilis, Chlamydien)
Contraindications
**Ceftriaxone must not be used at:* *
- Heavy hypersensitivity to cephalosporins
- Newborn with hyperbilirubinemia
- Newborns receiving calcium infusions
Preview at:
- Penicillin allergy (about 10% cross allergy)
- Kidney failure
- Liver disease
- Gallensteinleid (can cause gallensludge)
pregnant: Can be used with appropriate indication. In Gonorrhoe in pregnancy, Ceftriaxon is a means of choice.
Style time: Go into breast milk in small amounts. Application possible.
Possible Side Effects
Acid (1-10%):
- diarrhea
- Pain at injection site
- Increased liver levels
- Skin rash
Occasionally:
- nausea, vomiting
- Headaches
- Mushroom infections (Mundsoor, vaginal)
- Blood-image changes
Selten:
- Allergic reactions
- Pseudomembranous colitis
- Gallenstein/Gallensludge
- Kidney stones
Jarisch-Herxheimer-Reaction: Fieber and deterioration may occur in the treatment of syphilis.
Interactions
Contracted in newborns:
- Calcium-containing infusion solutions (possible for urgent precipitation)
Reactions:
- Anticoagulants – increased bleeding tendency
- Aminoglycosides – synergistic effect (if severe infections are desired)
- Sample ecid – increased ceftriaxone levels
No interactions with:
- Hormonal contraception
- Most oral medicines
