What is Avamys® (Fluticasonfuroate)?

Avamys® is a nasal corticosteroid spray with the active ingredient Fluticasonfuroat. It is one of the most effective treatments for allergic rhinitis and is considered as a gold standard for moderate to severe hay fever.

Fluticasonfuroate is a newer corticosteroid with a particularly high binding affinity to the glucocorticoid receptor and a very low systemic absorption. This means: strong local effect in the nose with minimal side effects in the rest of the body.

Avamys® is also characterized by a pleasant spray profile – the fine spray mist is evenly distributed in the nose and causes less irritation than other nasal sprays.

Active Ingredients & Mechanism of Action

Active ingredient: Fluticasonfuroat

Fluticasonfuroate acts as a strong anti-inflammatory corticosteroid in the nasal mucosa:

Active mechanism:

  • Ties to the glucocorticoid receptor in the cells of the nasal mucosa
  • Inhibits the release of inflammatory mediators (prostaglandins, leukotrienes, cytokines)
  • Reduces the immigration of inflammatory cells (eosinophiles, mast cells, lymphocytes)
  • Reduces swelling of the nasal mucosa

Activation of symptoms:

  • Blocked nose (main advantage over antihistamines)
  • Nostrils
  • Nope
  • Itching
  • Also eye symptoms are often improved (naso-ocular reflex)

Initiation of action: A noticeable improvement often occurs after 8 hours. The full effect develops over several days of regular application.

Who is it suitable for?

Avamys® is suitable for:

  • Adults and young people from 12 years old with allergic rhinitis
  • Children from 6 years (under medical supervision)
  • Seasonal and perennial allergic rhinitis
  • Moderate to severe symptoms, especially nasal constipation

Especially recommended at:

  • Blocked nose as main symptoms (antihistamines are weaker here)
  • Inadequate effect of oral antihistamines alone
  • Combination with oral antihistamines in severe symptoms
  • Long-term treatment of perennial allergies

Not suitable:

  • Acute nasal sinus infection (not without medical treatment)
  • Nose bleeding or nasal injuries
  • After nose surgery (until complete healing)

Available Dosages

Adults and young people aged 12:

  • Start dose: 2 spray taps per nostril once a day (= 55 μg × 2 = 110 μg total)
  • Maintenance dose for symptom control: 1 spray per nostril once a day (55 μg total)

Children 6-11 years:

  • 1 spray per nostril once a day (55 μg total)
  • In case of insufficient control: 2 spray taps per nostril (medical control)

Available strength:

  • 27.5 μg Fluticasonfuroate per spray jet (120 spray jets per bottle)

Application duration:

  • Seasonal: throughout the pollen season
  • Ideally start 1–2 weeks before pollen start
  • Perennial: All year possible under medical control

How to Take

Application:

  • Nose gently sneaking before use
  • Shake a bottle
  • Insert spray opening into the nose hole, slightly inclined to the outside
  • Gently breathe through the nose during spraying
  • Do not spray the nose

Important notes:

  • Apply once a day, always at the same time
  • Regular application is crucial - not only if necessary
  • Pre-application: Spray head 6 times pump (priming)
  • After 30 days Non-use: Renewed primen
  • Clean spray head regularly

Quality:

  • Do not put the head in the neck
  • Spray easily directed away from the nasal wall
  • Don't sneak at the same time - gently breathing

Contraindications

**Avamys® must not be used at:* *

  • hypersensitivity to Fluticasonfuroate or any of the other components

Preview at:

  • Heavy liver failure (increased systemic exposure)
  • Simultaneous treatment with Ritonavir or other strong CYP3A4 inhibitors (increased cortisol suppression)
  • Active or latent tuberculosis
  • Untreated fungal infection of the nose
  • Recent nose surgery or nose injury
  • children (long-term use under medical control due to growth)

pregnant and breastfeeding: Should only be applied after strict use-risk weighing. Systemic admission is minimal, yet medical consultation is recommended.

Possible Side Effects

Family (1–10 %):

  • Nose bleeding (Epistaxis) – most common side effect
  • ulcer of the nasal mucosa

** Occasionally (0.1-1 %):* *

  • Headaches
  • Nose pain or discomfort
  • Nasent dryness

Selten:

  • hypersensitivity reactions (Urtikaria, Angioedema)
  • Anaphylaxis (extremely rare)
  • Nose septumperforation (for long-term use, very rare)

Systemic side effects: Systemic cortisone effects (weight increase, osteoporosis) are not to be expected when used as intended, since the systemic bioavailability of Fluticasonfuroat nasal is less than 0.5%.

** Nose bleeding occurs more frequently when the spray is sprayed directly onto the nostril wall. Proper application technology significantly reduces this risk.

Interactions

Preview with simultaneous use of:

  • Ritonavir and other strong CYP3A4 inhibitors (increasing systemic floodicason exposure, cushing risk)
  • Ketoconazole, Itraconazole (similar mechanism)
  • Other nasal or inhalative corticosteroids (additive cortisol suppression)

No relevant interaction with:

  • Oral antihistamines (safe and recommended combination)
  • leukotriene antagonists
  • Swelling nasal sprays (short time)

** The combination of Avamys® with an oral antihistaminic agent (e.g. Desloratadin) is standard therapy for moderate to severe symptoms.
