What is Testotop?
Testotop® is a testosterone gel for external use in adult men with proven testosterone deficiency. It contains testosterone and is applied to the skin once daily to restore physiological hormone levels.
Testotop® complements the available transdermal testosterone products and offers an alternative to Testogel®, Testavan®, and Tostran®.
Active Ingredients & Mechanism of Action
Active ingredient: Testosteron
Mechanism of action:
- Testosterone absorbed transdermally through the skin
- Restoration of normal androgen levels
- Action via androgen receptors in muscle tissue, bone, the CNS, and the genital organs
Activity profile:
- Even release throughout the day
- Stable blood levels after a few days of regular use
- Clinical improvement of symptoms after 2–12 weeks
Effects: Improvement in libido, erectile function, energy levels, muscle mass, and bone density.
Who is it suitable for?
Testotop® is suitable for:
- Adult men with confirmed hypogonadism
- As an alternative if other testosterone gels are not tolerated or preferred
Not suitable:
- Women and children
- Prostate cancer or breast cancer in men
- Severe organ failure (heart, liver, kidney)
- Polycythemia
- Active wish to have children
Available Dosages
Dosage:
- Recommended starting dose: 125 mg testosterone (5 g gel) once daily
- First check of the testosterone blood level about 1 week after starting, in the morning before the next application
- Adjustment by the doctor: reduction from 125 mg to 62.5 mg (2.5 g gel) if the level is above the normal range, or an increase of 62.5 mg (2.5 g gel) if it is below
- Maximum dose: 250 mg testosterone (10 g gel) per day
Monitoring:
- Regular blood level checks
- Monitor hematocrit, PSA, and liver function tests
- Target: testosterone in the mid-normal range
How to Take
Application:
- Once daily, preferably in the morning
- Apply to clean, dry, intact skin
- Recommended application areas as stated in the package insert
- Wash hands thoroughly after applying
Important notes:
- Avoid skin contact at the application site with women and children
- Apply at least 2 hours before showering/swimming
- Cover the area with clothing
- Do not apply to damaged or inflamed skin
Contraindications
Testotop® must not be used in cases of:
- Prostate cancer or suspected prostate cancer
- Breast cancer in men
- Hypersensitivity to testosterone
- Polycythemia
Use with caution in:
- Severe heart, liver or kidney failure
- Sleep apnea
- BPH
- Epilepsy
- Diabetes mellitus
- Cardiovascular disease
Possible Side Effects
Common (1–10%):
- Skin irritation at the application site
- Erythrocytosis / increased hematocrit
- Increase in PSA
- Acne
Uncommon (0.1–1%):
- Mood swings
- Gynecomastia
- Weight gain
- Hair loss
- Headache
Rare:
- Polycythemia
- Elevated liver function tests
- Worsening of sleep apnea

Yes, switching from testosterone injections (e.g., Nebido®) to a gel is possible. The switch should be supervised by a doctor, as the timing and the starting dose must be determined individually.
