What is fosfomycin?
Fosfomycin is a broad spectrum antibiotic used specifically for the one-off treatment of uncomplicated urinary tract infections (blasenitis). A single dose of 3 grams is enough to fight the infection.
The drug concentrates strongly in urine and reaches bactericidal concentrations there for several days. Easy intake and good efficacy make fosfomycin the preferred treatment of acute bladder inflammation in women.
Known brand names are Monuril® and Generika.
Active Ingredients & Mechanism of Action
Active ingredient: Fosfomycin-Trometamol 3 g
Fosfomycin acts bactericidal through a unique mechanism:
Active mechanism: Fosfomycin inhibits the enzyme MurA (pyruvyltransferase), which is necessary for the first step of cell wall synthesis of bacteria. Without a functional cell wall, the bacteria burst.

- Unique mechanism of action – no cross resistance with other antibiotics
- Very good urine concentrations (up to 48-72 hours bactericide)
- Effective against the most common pathogens of urinary infections
- Low resistance development
Activity spectrum:
- Escherichia coli (the most common pathogen of bladder inflammation)

- Clebsiella
- Enterokokken
- Proteus
Who is it suitable for?
Fosfomycin is suitable for:
- Women with uncomplicated acute cystitis
- As a one-time treatment with typical symptoms
- In case of first or rare occurrence
Type symptoms:
- Burning in water
- Frequent urinary rank
- Little urine despite urge
- Underlying pain
Not suitable for:
- Complicated urinary path infection
- Pyelonephritis
- Men (other treatment required)
- Children under 12 years
- Heavy kidney failure
Available Dosages
Standard dosing:
- 3 g of fosfomycin rometamol as a single dose
Depletion form:
- Granules for producing a solution for taking in
- Bag with 3 g of fosfomycin
For recurrent infections: Repetition after 3 days (medical decision).
How to Take
Preparation: 1. Dissolve bag contents in a glass with water (not hot) Two. Stirring until dissolved 3. Drink now
Time:
- Best night before bed
- On sober stomach (2-3 hours after eating)
- Or in the morning after getting up (2-3 hours before breakfast)
Why on empty stomach? Food slows up intake and can reduce urine concentration.
After receipt:
- Drink a lot to rinse the bubble
- Symptoms can still last 2-3 days
- In case of deterioration or fever visit doctor
Contraindications
**Fosfomycin must not be taken at:* *
- hypersensitivity to fosfomycin
- Heavy renal insufficiency (Kreatinin-Clearance <10 ml/min)
- Hemodialysis
**Not suitable for treatment at:* *
- Complicated urinary path infection
- Kidney pelvic inflammation
- Men
- Children under 12 years
pregnant: Can be used with appropriate indication. There is no evidence of damage. Individual case decision.
Style time: Fosfomycin goes into breast milk. At one dose, consider short silence.
Possible Side Effects
Acid (1-10%):
- diarrhea
- nausea
- Headaches
- Squeeze
** Occasionally (0,1-1%):* *

- stomach pain
- vaginal yeast infection
- Skin rash
Selten:
- Allergic reactions
- Increased liver levels
** The side effects are usually mild and temporary. Diarrhea is the most common side effect.
Interactions
Reduction reduced by:
- Metoclopramide - can accelerate gastric emptying
- Food – therefore sober to take
No clinically relevant interactions with:
- Hormonal contraception
- Most other medicines
