What is fluicason?

Fluticason generics contain the active ingredient Fluticasonpropionate and are active ingredient-like with the original preparation Flutide. They are used as basic therapy for long-term treatment of asthma bronchiale.

Fluticason is one of the most potent inhalative corticoids and is used in lower doses than Budesonid or Beclometason. It has low oral bioavailability and therefore little systemic side effects.

Available, for example, as Fluticason Cipla® or as combination preparation Salmeterol/Fluticasonpropionate. When combined with Salmeterol, it acts like seretides.

Active Ingredients & Mechanism of Action

Active ingredient: Fluticasonpropionat

Fluticasonpropionate has a strong anti-inflammatory effect:

Active mechanism: 1. Binding with high affinity of glucocorticoid receptors Two. The complex migrates into the cell nucleus 3. Activates anti-inflammatory genes 4. Inhibits proinflammatory cytokines

Pharmacological benefits:

  • Highest receptor affinity among ICS
  • Oral bioavailability < 1% (almost only local effect)
  • High lipophilia – good retention in lung tissue
  • Strong and persistent anti-inflammatory effect

equivalence doses: 100 μg Fluticason ≅200 μg Budesonid ≅200 μg Beclometason

Who is it suitable for?

Fluticason generics are suitable for:

  • Asthma bronchiale all heavy grades
  • Adults and children (depending on formulation)
  • As a basic therapy (controller)

Advantages of generic medicine:

  • Same active ingredient as tidal
  • Cost-effective
  • Proven effectiveness

Not suitable:

  • For the acute treatment of breathlessness
  • In patients under Ritonavir therapy (CYP3A4 interaction!)

Available Dosages

Fluticason Cipla / Generika:

  • 50 μg, 125 μg, 250 μg per stroke

Salmeterol/Fluticasonpropionate (combination):

  • 50/100 μg, 50/250 μg, 50/500 μg per dose

Dosing with asthma (adults):

  • Easy: 100-250 μg 2x daily
  • Medium: 250-500 μg 2x daily
  • Heavy: 500-1000 μg 2x daily

Equivalence doses note: Fluticason is twice as potent as Budesonid!

How to Take

Dosieraerosol: 1. Remove protective cap, shake Two. Deep exhalation 3. Close the mouthpiece 4. slowly inhale, triggering spray Five. Keep breath 10 seconds 6. Rinse and sprinkle

Powder inhaler (if available):

  • Breath controlled – vigorous breathing
  • rinse mouth after inhalation

Important:

  • Regular use, 2x daily
  • rinse mouth after each inhalation
  • Do not drop abruptly

Contraindications

Do not apply at:

  • Hypersensitivity to Fluticason
  • Simultaneous therapy with Ritonavir or Cobicistat (Cushing Risk!)

Preview at:

  • Active lung tuberculosis
  • Mushroom infections of the respiratory tract
  • Heavy liver failure

pregnant: Can be used with asthma.

Style time: Application possible. Low systemic exposure.

Possible Side Effects

Acid (1-10%):

  • Mundsoor
  • Heiserkeit
  • Rache irritation

Occasionally:

  • Headaches
  • Taste disorders

Selten (at high doses):

  • Adrenal suppression
  • osteoporosis
  • Cataract, glaucoma
  • slowing growth in children

Important: Special caution at CYP3A4 inhibitors – systemic floodicason mirrors can dramatically increase!

Interactions

Critical interaction:

  • Ritonavir, Cobicistat – CONTRAINDICES! Highly elevated systemic Fluticason mirror, Cushing syndrome possible
  • Ketoconazole, Itraconazole – raised mirror

For HIV therapy: Avoid Floicason! Alternative: Beclometason.

No relevant interaction with:

  • Bronchodilatators
  • Antihistamines
