What is Berotec?
Berotec is a short-acting beta-2 sympathomimetic (SABA) containing the active ingredient fenoterol. It is used as an emergency medication for acute shortness of breath caused by asthma or COPD.
Fenoterol works similarly to salbutamol but has a somewhat stronger effect on the cardiovascular system. Berotec is mainly used as a metered-dose inhaler and as a solution for nebulizers. The active ingredient is also contained in the combination product Berodual (with ipratropium).
Berotec is manufactured by Boehringer Ingelheim.

Fenoterol acts as a beta-2 agonist:
Mechanism of action:
- Activates beta-2 adrenoceptors in the bronchial muscles
- Increases intracellular cAMP levels
- Relaxes smooth muscle
- Widens the narrowed bronchi
Time course:
- Onset of action: 3-5 minutes
- Maximum effect: 30-60 minutes
- Duration of action: 3-5 hours
Comparison with salbutamol: Fenoterol has a somewhat lower beta-2 selectivity and therefore has a stronger effect on the heart. Salbutamol is preferred for most patients.
Who is it suitable for?
Berotec is suitable for:
- Acute shortness of breath in asthma or COPD
- As-needed treatment of bronchospasm
- Prevention of exercise-induced asthma
Not suitable as:
- Sole long-term treatment for asthma
- A substitute for inhaled corticosteroids
Note: Salbutamol is the first-choice SABA for most patients. Berotec is an alternative in cases of intolerance or as a component of Berodual.

Berotec N metered-dose inhaler:
- 100 µg fenoterol per puff
- As needed: 1-2 puffs
- Maximum: 8 puffs per day
Inhalation solution (nebulizer):
- 0.5 mg/ml
- Acute: 0.5-1 ml diluted with NaCl

How to Take
Metered-dose inhaler: 1. Remove the protective cap, shake 2. Breathe out deeply 3. Close your lips around the mouthpiece 4. Breathe in slowly while releasing a spray 5. Hold your breath for 10 seconds 6. Breathe out slowly
For acute shortness of breath:
- 1-2 puffs, repeat after 5 minutes if needed
- For severe shortness of breath, call emergency medical services immediately!
Contraindications
Do not use in cases of:
- Hypersensitivity to fenoterol
- Hypertrophic obstructive cardiomyopathy
- Tachyarrhythmias
Use with caution in cases of:
- Severe heart disease
- Overactive thyroid
- Pheochromocytoma
- Diabetes mellitus
- Hypokalemia
Pregnancy: Only if clearly indicated. Fenoterol can inhibit labor contractions.

- Tremor
- Dizziness
- Nausea
Uncommon:
- Muscle cramps
- Heart rhythm disorders
- Hypokalemia
- Paradoxical bronchospasm

- Racing heart, palpitations
- Headache
- Nervousness
Interactions
Interactions:
- Beta blockers – may cancel out the effect of fenoterol
- MAO inhibitors – increased effect on the heart
- Tricyclic antidepressants – increased cardiac effects
- Diuretics, corticosteroids – risk of hypokalemia
