What is Telmisartan?

Telmisartan is an AT1 receptor blocker with the longest half-life of all sartans (about 24 hours). This means a particularly uniform 24-hour blood pressure protection, even in the critical early hours.

Known brand names are Kinzalmono® and Micardis®. Telmisartan is also available in combination with hydrochlorothiazide (Micardis® Plus). It also has a mild PPAR-gamma activation, which can have a positive effect on metabolism.

Active Ingredients & Mechanism of Action

Active ingredient: Telmisartan

Telmisartan blocks the AT1 receptor particularly long-lasting:

Active mechanism:

  • High-selective and long-lasting AT1 blockade
  • Longest half-life of all Sartane (about 24 hours)
  • Lipophile – high tissue penetration

Additional properties:

  • Mild PPAR-gamma activation (substance-promoting)
  • Can easily improve insulin resistance
  • Cheap effects on lipid metabolism

Clinical meaning: In the ONTARGET study, Telmisartan was equivalent to the ACE inhibitor Ramipril in high-risk patients.

Who is it suitable for?

Telmisartan is suitable for:

  • Adults with high blood pressure
  • cardiovascular prevention in high risk patients
  • Patients who need a reliable 24-hour action

Especially recommended at:

  • Early blood pressure (low blood pressure)
  • Metabolic syndrome
  • Patients who do not use Ramipril

Not suitable:

  • Pregnancy and breastfeeding
  • Gallenwegsobstruction
  • Heavy liver failure

Available Dosages

High blood pressure:

  • Start dose: 40 mg once a day
  • usual dose: 40-80 mg once a day
  • Maximum dose: 80 mg daily

Cardiovascular prevention:

  • 80 mg once a day

Available starches:

  • 20 mg, 40 mg, 80 mg tablets

Combination:

  • Telmisartan + Hydrochlorothiazid (Micardis® Plus)

How to Take

Intake:

  • Once a day, if possible in the morning
  • Independent meals
  • Take with water
  • Do not share or chew tablet

  • Regular blood pressure controls
  • Laboratory controls (animal, potassium)
  • Do not drop abruptly

Contraindications

**Telmisartan must not be taken at:* *

  • hypersensitivity to Telmisartan
  • Pregnancy and breastfeeding
  • Gallenwegsobstruction
  • Heavy liver failure
  • At the same time with aliens in diabetes

Preview at:

  • Kidney failure
  • Kidney artery stenosis
  • Hyperkaliemia
  • Primary hyperaldosteronism

pregnant: Strikt contraindicates throughout pregnancy.

Possible Side Effects

  • Squeeze
  • urinary tract infections
  • infections of the upper respiratory tract
  • Back pain
  • Gastrointestinal complaints

Selten:

  • Hyperkaliemia
  • Kidney function reduction
  • Angioedema
  • Liver malfunction

Good tolerated: Telmisartan has shown in studies a compatibility that is hardly above placebo levels. It is one of the best tolerated blood pressure medications.

Interactions

Contracted:

  • Aliskiren in diabetes or kidney failure

Preview at:

  • Potassium-saving diuretics → Hyperkaliemia
  • NSAR → reduced effect
  • Lithium → raised mirrors
  • Digoxin → mirror increase possible

Fabric change: Telmisartan is not degraded via CYP enzymes, so little interactions about this path.
