What is Priligy?

Priligy is the first and only medicine authorised in Germany specifically for the treatment of premature sperm (Ejaculatio praecox). The active ingredient Dapoxetin belongs to the group of selective serotonin reuptake inhibitors (SSRI), but was developed specifically for this indication.

Unlike other SSRIs, which are taken daily as antidepressants, Dapoxetin acts very quickly and is only taken if necessary. The effect already begins with the first intake – no need for weeks of saturation.

In clinical trials, Priligy was able to extend the time to sperm by 2 to 3 times. Many men also report on improved control and higher satisfaction with sexual life.

The drug is distributed by Menarini and is available in Germany since 2009.

Active Ingredients & Mechanism of Action

Active ingredient: Dapoxetin

The background: The premature sperm has often to do with an imbalance of the messenger substance serotonin in the brain. serotonin plays an important role in controlling ejaculation – higher serotonin levels can inhibit ejaculation reflex.

The effect of Dapoxetin: Dapoxetin inhibits the resumption of serotonin into the nerve cells. As a result, more serotonin remains available in the synaptic gap and the signal transmission is changed. This leads to better control over the ejaculation time.

What makes Dapoxetin special:

  • only if necessary, not daily
  • Effect already at first intake
  • No accumulation in the body

Study results: The average time until sperm was extended from about 1 minute to 3-4 minutes. More than 50% of men reported a significantly improved control.

Who is it suitable for?

Priligy is suitable for men with:

Primary (lifelong) premature sperm:

  • The problem has existed since the first sexual experiences
  • Often genetic or neurobiological
  • Typical time to ejaculation: under 1-2 minutes

Acquired premature sperm:

  • The problem occurred only during life
  • Often associated with stress, relationship problems or erection disorders
  • Start after a phase of normal ejaculation control

Important conditions:

  • Age 18-64 years
  • Diagnosis of premature sperm
  • Significant suffering pressure
  • No serious heart disease
  • No use of certain drugs (see contraindications)

**Priligy can be combined with:* *

  • PDE-5 inhibitors (Sildenafil, Tadalafil) with simultaneous erection disorder
  • The doctor should monitor this combination

Available Dosages

available dosages:

  • 30 mg – Recommended initial dose

Dosing recommendations:

  • Start: 30 mg, 1-3 hours before sexual intercourse
  • In case of insufficient effect: Increase to 60 mg possible
  • For severe side effects: Stay at 30 mg or rethink therapy

Important:

  • Maximum one dose per 24 hours
  • Only take on scheduled intercourse
  • Not suitable for daily continuous intake

First assessment: After 4 weeks or 6 receipts, the treatment success should be checked. If there is no sufficient effect, the therapy should not be continued.

How to Take

Intake:

  • 1-3 hours before scheduled intercourse
  • Swallow tablet with a glass of water
  • With or without food possible
  • Not more than once a day

Optimal time: The effect is strongest about 1-3 hours after taking. Plan accordingly, but do not put yourself under pressure – Priligy also works if taking takes place slightly earlier or later.

Alcohol: The combination with alcohol is not recommended. Alcohol enhances the risk of side effects such as dizziness, anxiety and powerlessness.

After receipt: Pay attention to dizziness or disorder, especially when you get up. Stand up slowly and sit down or lie down when you feel uncomfortable.

Contraindications

Absolute contraindications:

Heart diseases:

  • Heart failure (NYHA II-IV)
  • severe heart rhythm disorders
  • coronary heart disease
  • Heart valve diseases

Psychical diseases:

  • Depression (also in prehistory)
  • Bipolar disturbance
  • Manie

Medical interactions:

  • MAO inhibitors (even until 14 days after decommissioning)
  • Other SSRI or SNRI
  • Tricyclic antidepressants
  • Triptans
  • Tramadol
  • St. John's wort
  • Strong CYP3A4 inhibitors (Ketoconazole, Ritonavir, etc.)

More contraindications:

  • Heavy liver failure
  • Powerfulness in prehistory
  • Uncontrolled epilepsy
  • Age under 18 or over 64 years

Preview at:

  • Low blood pressure
  • Orthostatic hypotension
  • Taking blood pressure cores

Possible Side Effects

**Family side effects (1-10%):* *

  • Squeeze
  • Headaches
  • nausea
  • diarrhea
  • tiredness, sleepiness
  • Sleep disorders
  • Nervosity

** Occasionally (0,1-1%):* *

  • Hidden vision
  • Mouth dryness
  • Sweating
  • Erection problems (paradox)
  • Blood pressure drop when standing (orthostatic hypotension)

Seltene, but important side effects:

  • Ohn power (synkope): Especially when standing up or in combination with alcohol. It is preferably in the first hours after taking. Immediately sit down or lay down with dizziness.
  • Serotonin Syndrome: In combination with other serotonic drugs. Symptoms: restlessness, trembling, sweating, fever – immediately consult doctor.

** Many side effects (irrigibility, dizziness) are dose-dependent and occur at 30 mg less frequently than at 60 mg.

Interactions

Absolute contraindicated:

  • MAO inhibitors: Heavy, potentially life-threatening interactions
  • Other SSRI/SNRI: Increased serotonin syndrome risk
  • Triptane: Increased serotonin syndrome risk
  • Tramadol: Increased serotonin syndrome risk
  • Starke CYP3A4 inhibitor: Ketoconazole, Itraconazole, Ritonavir

Important interactions:

  • Moderate CYP3A4 inhibitor (erythromycin, fluconazole): maximum dose 30 mg
  • CYP2D6 inhibitors: Caution, Dapoxetin mirror can be increased
  • PDE-5-Hemmer: Combination possible but close-mesh monitoring
  • Blow pressure medication: Increased blood pressure reduction possible
  • Alcohol: Increases risk of powerlessness and side effects

Johanniskraut: Do not take at the same time – increased serotonin syndrome risk.
