What is Doloproct® (Fluocortolon/Lidocain)?
Doloproct® is a prescription combination preparation for treating hemorrhoids and other inflammatory diseases of the anal region. It contains two fluocortolon ester (fluoroocortolon pivalate and fluocortolonhexanoate) as a corticosteroid component and Lidocain as a local anesthetic.
The combination of two fluocortolon esters with different duration of action ensures rapid action (pivalate) and a sustained action (hexanoate). Lidocain anesthetizes the affected area and relieves pain and itching immediately.
Doloproct® is available as rectal ointment and as suppositories and is used in hemorrhoids grade I–II, anal fissures, anal eczema and proctitis.
Active Ingredients & Mechanism of Action
Active ingredient: Fluocortolonpivalat / Fluocortolonhexanoat / Lidocainhydrochlorid
Doloproct® has two mechanisms:
1. Fluocortolon (Kortikosteroid):
- Strong local anti-inflammatory effect
- Two esters with different duration of action:
- Fluocortolonpivalat: Fast action
- Fluocortolonhexanoate: Long-lasting action
- Reduces swelling, redness and inflammation
- Inhibits the release of inflammatory mediators
- Reduces itching and burning
2. Lidocain (local anesthetic):
- Blocks the sodium channels of the nerve fibers
- Immediate local anesthesia
- relieves pain, burning and itching
- Effects within minutes
- duration of action: 1–2 hours
Combined action: Lidocain immediately relieves the acute symptoms, while Fluocortolon treats inflammation and ensures continued improvement.
Who is it suitable for?
Doloproct® is suitable for:
- Hemorrhoids degree I and II
- Analeczem (Ekzem in the anal area)
- Analfissures (superficial tears)
- Proctitis (inflammation of the end intestine)
Especially suitable for:
- Acute complaints with pain and itching
- inflammatory hemorrhoid strokes
- Short-term treatment of anal complaints
Not suitable:
- Hemorrhoids degree III–IV (medical treatment/OP necessary)
- Mushroom or virus infections in the anal area (e.g. herpes, candidose)
- Tuberculosis or syphilis in scope
- Long-term application (max. 2–4 weeks)
Available Dosages
Rectal ointment:
- apply 2–3 × daily thin
- The first day may be more frequent (up to 4 ×)
- After improvement: reduce dose to 1 × daily
- Application time: max 2–4 weeks
Suppositories:
- 1 cups 2–3 × daily introduce rectal
- The first day up to 4 × daily possible
- After improvement: 1 × daily
- Application time: max 2–4 weeks
Important:
- Apply no longer than 4 weeks uninterrupted
- Reducing dose and frequency step by step
- Do not drop abruptly after prolonged use

How to Take
Rectal ointment:
- Before application gently clean the anal region
- apply thin layer to the affected area
- In inner hemorrhoids: screw applicator on the tube and insert ointment rectally
- After application wash hands
Chapters:
- Introduction after the chair and cleaning
- Dissolve cups from the packaging
- Insert into the after (rounded end forward)
- Apply as possible after stool
Important notes:
- Not with open, wet wounds without medical instruction
- In case of ongoing complaints after 1 week: Medical control
- Do not combine with latex condoms (Lidocain can damage latex)
- No long-term application due to skin atrophiic risk (corticosteroid)
Contraindications
**Doloproct® must not be used at:* *
- hypersensitivity to fluocortolone, lidocaine or other components
- Virus infections in the field of application
- Mushroom infections (candiosis) in anal area
- Tuberculosis or syphilis in scope
- Bacterial skin infections (Impetigo) in the area
Preview at:
- Diabetes mellitus (Corticosteroid can affect blood sugar)
- Long-term application (Hautatrophie, Teleangiectasis)
- Pregnancy and breastfeeding (only after medical weighing)
- Large-area application (increased systemic recording)
pregnant: Only after strict use-risk weighing, especially in the 1st trimester. Do not apply over a long period of time.
Possible Side Effects
** Occasionally (0.1-1 %):* *
- Burning at the application site
- Itching at the application site
- Contact dermatitis
For long-term use:
- skin atrophie (skin dilution)
- Teleangiectasia (visible small blood vessels)
- Striae (stretching strips)
- Perianal dermatitis
- Increased susceptibility to infection in the scope of application
Selten:
- allergic reactions to lidocaine or fluocortolon
- Systemic cortisone effects (only with very large-area/long-term use)
** For normal short-term application (max. 2–4 weeks), side effects are rare and usually mild. The skin atrophie travel is mainly used for longer periods.
Interactions
Clinical interactions: Systemic interactions are unlikely in the case of proper rectal application, since systemic uptake is low.
Theoretically possible:
- Other local anaesthetics: additives
- Class I Antiarrhythmics: Theoretical amplification by Lidocain
- Latex condoms: can be damaged by the ointment base
** Due to the local application and low systemic absorption, clinically relevant drug interactions at Doloproct® are extremely rare.
